Les processus opérationnels de healthdata.be

Lors de la phase de lancement de la plateforme healthdata.be (mars 2014), il a été constaté que chacun des projets d’enregistrement existants connaissait sa propre genèse et suivait son propre cycle de vie. Malgré les nombreuses similitudes en termes d’objectifs et d’acteurs, il n’existait pas d’approche standardisée pour le lancement et la gestion d’un projet d’enregistrement.

AideDesAutres

There are often multiple people or parties involved in the care for a patient with a disorder or disability, particularly in the event of home care. Their efforts enable the patient to function more or less independently. This includes not only caregivers, but also professional help such as that offered by home care organizations, nurses, aids and helpers. Current doctors and staff from the facility to which a patient is admitted do not fall under this concept.

L' architecture technique de healthdata.be

L’équipe healthdata.be a mis au point, après différentes consultations avec les représentants des services ICT des acteurs de soins ainsi que de la plateforme eHealth, une architecture de base en vue de la collecte et de la mise à disposition des données à caractère personnel codées concernant la santé.

Mobility describes the abilities and any limitations in all aspects of mobility, such as changing position, walking and moving forward, in some cases with the help of an aid.

MarqueMUST

The Malnutrition Universal Screening Tool (MUST) is a (validated) diagnostic screening tool for determining the extent of a patient’s malnutrition. For this screening tool, every patient’s Body Mass Index (BMI) and weight loss percentage is calculated, after which the patient is assigned a disease factor. The MUST helps to recognize and treat malnutrition at an early stage.

DevelopmentChild

Information on the development and growth of a child is important in determining the kind of care that should be provided to the child and the family. When the child is being transferred, continuity of care can be achieved . Information can provide insight in the process of development and whether the child has a developmental delay.

ParticipationALaSociété

Participation in society pertains to the patient’s initiative to participate in social activities in daily life, such as practicing a profession and hobbies. Participation in society also describes the patient’s social network.

Déploiement HD4DP: l'état

Depuis Juin 2015, l'équipe de healthdata.be, ensemble avec les services TIC des hôpitaux et des laboratoires, a commencé l'installation du HD4DP logiciel. La situation (1er Septembre 2016) est rapporté ci-dessous.

SNAQ65+Score

The Short Nutritional Assessment Questionnaire 65+ (SNAQ65+) is a (validated) measuring tool for determining the extent of a patient’s malnutrition. The tool comprises a number of questions, each of which is assigned a score. The total score determines the extent of malnutrition. The SNAQ helps to recognize and treat malnutrition at an early stage. Three varieties of the measuring tool were developed for different care settings: hospitals (adults and children), nursing and retirement homes and independently living seniors. This concept describes the tool intended for the setting of independently living elderly.
